Common PIF Errors - Anesthesiology
- Incorrect program name (use name listed in GME Directory)
- Omission of 10 digit program number
- PIF is not dated (upper right hand corner of first page)
- Missing signatures (The signatures of the Program Director, the Department Chair, if different from the Program Director, and the Designated Institutional Official are required)
- Incorrect pagination: All pages, including inserts and attachments should be consecutively numbered. The pagination should correspond to the inclusive numbers in the Table of Contents.
- Missing affiliation agreements. Agreements not signed by both parties
- Information omitted and questions left unanswered
- Inconsistent data, especially the number of resident positions currently filled/requested
