Integrating Clinical Practice, QI, and the Competencies
into the M&M Conference

Purpose: We use the M&M
to establish an educational and professional expectation that
residents need to take an active role in practice-based learning
and improvement and systems-based practice by assessing clinical
practice, patient safety and the quality of care provided
by individual practitioners, the residency program, and the
institutional system. The M&M conference is an excellent
tool for incorporating all of the ACGME competencies into
a learning opportunity that also promotes leadership, research,
and scholarly activity.
Methodology :
M&M Worksheet used by residents & attending physician to identify an appropriate clinical issue. Residents identify all of the competencies involved with the issue, possible root causes, and suggestions for improvement
Faculty mentors assist residents in preparing a professional presentation that compares evidence-based medicine to actual clinical practice, identifies gaps in medical knowledge, and coaches the team on how to lead and focus the discussion
Residents are limited to a maximum of 12 PowerPoint slides for a 45-minute discussion to encourage interpersonal communication skills
Residents critically evaluate peer practice using a Peer Review tool similar to the form used by our institutional Physician Performance Improvement Committee
Senior resident for the team presents a summary of the case & suggestions for improvement at the monthly program QI meeting to promote leadership skills and active involvement with professional medical committees
The program QI committee includes all clinical faculty, the program director, the DIO, the institutional QI directors, the residents involved with the case, and invited guests specific to the issue being discussed
The DIO and QI directors are responsible for taking suggestions
for institutional system changes to the appropriate individual/department.
Our residency program is an integral part of the institutional
QI program.
Results: We implemented our
QI focused M&M at the beginning of the 2005-2006 academic
year. We have actually implemented, or are in the process
of implementing changes for every M&M conference presented
since September 2005. Our M&M conferences provide a safe
venue for residents to identify areas of improvement, and
promote professionalism, ethical integrity and transparency
in assessing and improving patient care.
