Scenario: Using portfolio development to aid learning of Systems-based Practice concepts.
As the director of a pathology residency program, you would like to address the general competencies by building on existing curricular activities. You believe that the laboratory management rotations provide residents a good opportunity to learn about Systems-based Practice concepts. During these rotations, residents are typically in charge of a laboratory, and need to understand systems issues in order to run operations smoothly.
The illustration below describes how portfolio development may be used to address Systems-based Practice during your laboratory rotation. Your learning objectives are that residents will be able to: 1) use systems concepts to describe the relationship between laboratory operations and hospital operations, and identify system weaknesses; 2) describe the resources used to complete specific laboratory tests; and 3) use appropriate codes to bill for laboratory tests.
Illustration:
You determine that asking residents to create and maintain portfolio entries of the following would help them learn about Systems-based Practice concepts: a flow chart of processes involved in processing laboratory requests that depicts system weaknesses; a log of resources used to conduct specific laboratory tests; and copies of billing forms completed by the resident for laboratory services. You provide samples of each of these entries to your residents so that they know what is expected of them. You schedule biweekly meetings with your residents, and use their portfolio entries to guide discussion about laboratory operations and resources. Following are examples of the systems issues you may address:
- Use the flow chart to examine the working relationship between the laboratory and hospital. As an example, the laboratory test request form could be examined. Is the form standard across units? Is it easy to complete and read? How might changes to the form affect the quality of patient care?
- Use the resource utilization log to stimulate discussion about weighing the costs of specific tests with their effectiveness for diagnosing specific diseases.
- Use the billing form to guide discussion about resource management in medicine.
