Advisory Commission on Additional Licensing Models Releases Second Set of Draft Recommendations
The Advisory Commission on Additional Licensing Models has released its second set of recommendations for review and comment to guide development and implementation of additional licensure pathways for internationally trained physicians (ITPs).
Building on its initial guidance released in February 2025, this set of recommendations focuses on assessment and supervision during the provisional licensure period. They aim to ensure that ITPs entering the US health care workforce are evaluated using a competency-based framework aligned with US standards, while supporting fair treatment and well-being during the transition.
The recommendations are designed to help align policies, regulations, and statutes across states, adding clarity and specificity to support public protection and improve access to high-quality care, particularly in rural and underserved communities. This guidance is especially relevant for states and territories that have enacted, introduced, or are considering legislation to create new licensure pathways for ITPs.
The Advisory Commission was formed in December 2023 by the Federation of State Medical Boards (FSMB), Intealth™, and the ACGME in response to increasing interest among state policymakers to improve patient access and reduce workforce shortages by changing licensure requirements for physicians who have completed training and/or practiced outside the US.
