NEW! Proceedings Paper from the Summit on Fostering Disability-Inclusive Health Care in Resident Education
The ACGME is excited to share the findings from the Summit on Fostering Disability-Inclusive Health Care in Resident Education. Held from March 2-4, 2025, this event brought together 100 physicians, disability education advocates, and various stakeholders to tackle the challenges in training physicians to deliver disability-inclusive care and to formulate practical, actionable solutions.
The deliberations, recommendations, and resources discussed at the Disability Summit have been compiled into a proceedings paper. This document provides guidance for program directors and others involved in designing curriculum and educational experiences for residents and fellows. While these insights are not requirements, they offer valuable reflections from the attendees, serving as a roadmap for curriculum planning and demonstrating the medical education community's commitment to disability-inclusive care.
NEW! 2025 Milestones Predictive Probability Value (PPV) Report
The 2025 Milestones Predictive Probability Value (PPV) Report is now available on the Research and Reports page of the Milestones section of the ACGME website.
This report presents the latest PPVs for specialties and subspecialties with sufficient data to allow for meaningful analysis. It is designed to help support the learning and development of residents and fellows.
PPVs estimate the likelihood that residents or fellows will not reach Level 4 by graduation, based on previous national longitudinal Milestone data collected before graduation. Presented in a heat map format, these values help identify individuals who may be at risk of falling behind. This enables timely intervention, the creation of personalized learning plans, and potential opportunities to improve the curriculum.
A beta testing initiative is underway to develop program-specific PPV reports tailored to each program’s unique context. Program directors—especially those from smaller programs—who are interested in previewing a beta version of their program’s PPV tables are encouraged to contact the Research and Evaluation Team at gmeresearch@acgme.org. The GME community’s feedback is valued and will help enhance future versions of the report.
The companion document, the Milestones National Report, can be found on ACGME Cloud | Analytics – Explore Public Data, within the Explore Milestones module.

NEW! JGME Articles of the Week
Assessing Intern Confidence in Entrustable Professional Activities at Onset and Conclusion of First Year of Training: A Longitudinal Observational Study
Yana Zemkova, MD; Jennifer Strouse, MD; Jane Rowat, MS; Clarence Kreiter, PhD; Manish Suneja, MD
The Association of American Medical Colleges developed 13 core entrustable professional activities (EPAs) physicians should proficiently perform with indirect supervision when starting residency. Despite efforts to integrate EPAs into undergraduate medical education, concerns persist regarding graduate preparedness, including first-year residents' self-confidence in performing EPAs. Using an observational study, the authors found that first-year resident confidence significantly improved across all core EPAs over the year, though notable gaps remained in areas such as procedures and system failures.
Quantum Thinking in Graduate Medical Education: Transforming Minds, Methods, and Possibilities
Krishna Mohan Surapaneni, PhD, MHPE
In this Perspectives article, the author argues that conventional models of teaching and learning struggle to keep pace with modern education and practices, and encourages readers to adopt a quantum thinking approach to reshape the questions being asked.
Leading Major Change in Graduate Medical Education: Initiate, Implement, and Sustain
Harm Peters, MD, MHPE; Dink Jardine, MD, FACS; Madeline Joseph, MD, FAAP, FACEP
This article presents a scenario in which an organization's incidents of inappropriate, unethical, or disrespectful behavior by learners and teachers have been reported more frequently. As professionalism sessions and discussions at the program and institutional levels have not had sufficient impact, a cultural change is needed. This Rip Out article covers the essential skills needed for GME leaders to successfully navigate such a challenge and drive meaningful, lasting change. Listen to the audio version of this article.
