Supervising Resident AI Use Without Losing the Learning
Carl Preiksaitis, MD, MEd
This issue of AI Teaching Rounds focuses on supervising residents who use artificial intelligence (AI) tools during clinical work and strategies to encourage clinical reasoning. The author recommends protecting core cognitive tasks, such as forming differential diagnoses, and using structured supervision strategies to ensure residents actively engage with and critically evaluate AI input. Effective supervision requires being intentional about when AI is used so it enhances, rather than replaces, the human thinking central to medical education and training.
Alan Schwartz, PhD, JD; Keia Hobbs, MD; Daniel J. Schumacher, MD, PhD, MEd
Harmonized ACGME Milestones were introduced in 2021 to enable cross-specialty comparisons, but it is unclear whether users interpret milestone levels consistently. A scaling study conducted on pediatric and internal medicine educators revealed that some harmonized milestone levels are not meaningfully distinguished by users and that perceptions vary by specialty and role. Differences in how milestone levels are understood may impede the effectiveness of harmonization for cross-specialty comparisons and communications.
